Anyone know a physician here in Phoenix, Mesa, Tempe, Scottsdale, that will prescribe Phentermine or Meridia?

My primary care physician is fairly heavy himself and so are several of his assistants and won't prescribe it. However, I quit smoking and gained 36 pounds and since I was already obese---now my Bmi is Over 41!!! All I want is a 30 day kick start to stop my grazing.
Anybody know a physician?
